The picture above shows a before and after of a tooth with a Sealant placed. As you can see in the first picture, the grooves are very narrow and one single tooth brush bristle is too wide to get into the groove to clean it.

Placing a Sealant, which fills in the grooves will prevent bacteria from entering in the first place.
